We provide referral assistance Monday
through Friday 8:00 a.m. to 7:00 p.m. We are happy to answer questions
from you or your patients about scheduling an appointment. We provide a
single point-of-contact, toll-free, help line for patient scheduling
and inquiries.
Our clinical staff is on hand 24 hours a day, 7 days a week to ensure
safe, comfortable sleep testing.
Scheduling sleep lab testing
Neurocare Sleep Centers are located in Newton, Brockton, Concord (Emerson Hospital), Concord-Pediatric
and Derry, NH
(Parkland Medical Center). Click on the town to download the
Request for Services form for the location that is most convenient for
your patients. Please complete the form and fax it to us.

Unless you prefer a specific Neurocare network sleep lab, we will
attempt to schedule testing at the facility that is most convenient for
your patient. If you would like an expert sleep consultation for your
patient, we can recommend specialists at our Newton, Massachusetts,
sleep center, or other Neurocare-affiliated specialists.
Please inform us of special risks or service
requirements. This is extremely helpful to all concerned and an
important aspect of risk management.
